Smile Source Location Updates
Welcome! Please use this page to add or remove any locations from the approved Smile Source list on file with Zyris
How to use this tool
- You can add/remove practices from the list – once added, you can’t edit from this tool
- Do not delete, sort, or filter rows
- Fill out rows one at a time, top to bottom
- Do not paste values into the document
- Values entered in columns “Main Contact Email” and “Practice General Email” should be unique to that Practice location only
- Be sure to enter the suite number, building number, etc. when applicable
- Do not edit the highlighted columns on the far right of the document. Once the information is synced with Zyris the Practice’s customer number will be inserted
- If you need to add a Practice location you have already marked with a removal date, enter the location again on a new line
- The data sync with Zyris will occur once a day, please plan orders accordingly
